The H.O.P.E. Foundation™ is a nonprofit organization committed to reimagining reunification by transforming how families are supported before, during, and after parent-child separation. We walk alongside families with intention, centering their voices, honoring their lived experiences, and building upon the strengths they already possess.

Through the H.O.P.E. Continuum Practice Model™, we provide a structured, healing-centered framework designed to support families across the full continuum of separation and reunification. Our approach recognizes that reunification is not simply an event or case outcome, it is a journey of healing, restoration, skill-building, and long-term stability.

Unlike traditional models that focus primarily on system navigation, the H.O.P.E. Continuum Practice Model™ combines healing and grief recovery support, family empowerment coaching, lived experience partnership, family restoration planning, and post-reunification support to address both the practical and emotional challenges families face throughout the reunification journey.

We partner with families experiencing both formal and informal parent-child separation, including those involved in foster care, kinship caregiving arrangements, and other circumstances where children and parents have become disconnected. By providing coordinated, family-centered support, we help families strengthen relationships, build protective capacities, and create sustainable pathways toward reunification and long-term family stability.

Our commitment does not end when children return home. Through Sustainability & Aftercare Support, we continue walking alongside families during one of the most critical phases of the journey, life after reunification. By helping families strengthen support networks, navigate challenges, and maintain progress over time, we promote safe, stable, and sustainable family outcomes.
